I installed the hook but it doesn't display in the forum. I only can display it in admincp. I think its a bug. I already send a support to the author but he not yet respond to it. Is there any other way of adding icons? Thank you :)

If you uploaded all files correctly...

After that, on your Admin CP, go to System tab and click on -> Applications & Modules -> Manage Applications & Modules. In the end of this page, on Applications Not Installed, you'll see (DP32) Forum Icons. Click on Install link and after on Continue and wait until the application is installed.

Still inside the Admin CP, visit the Other Apps tab. There will be a section here entitled (DP32) Forum Icons. Click on it and have fun with this application!

Your ACP page looks very strange.
There is nothing listed under "System" tab except for changing email address and password.
Is it legit IPB installation?

You had one setting wrong (Display Mode).
I corrected that but it didn't help.

I suggest you post in hook's support topic.
